We are continually striving to meet and exceed our patient’s needs and expectations. After receiving service from CNY Infusion Services, we will contact you for a service evaluation. This is an example of the questions we may ask:

1) Was our role in your home infusion experience thoroughly explained to you prior to our providing services?

2) Did we meet your needs and expectations in the provision of services as we promised?

3) Was our staff skilled and knowledgeable about your therapy?

4) Would you use our agency again for your home infusion needs?

5) Was your nurse or pharmacist sensitive to your needs and did they communicate effectively with you?

6) Did you feel safe receiving your infusion service at home; and did you feel there is any way we could have improved your safety?
